REGISTRATION POLICIES: Payment must accompany registration form. REGISTRATIONS RECEIVED BY OCTOBER 8, 2004 WILL BE PROCESSED AND MAILED OUT TO U.S. REGISTRANTS ONLY. Registration forms received after October 8 will require registrant to pick up badge and tickets on-site at the Convention during registration hours. Cancellations received prior to October 15, 2004 entitle registrant to full refund. Cancellations after that date will be subject to a 20% processing charge. No refunds will be made unless written request is received by November 11, 2004. No refunds will be made on ticketed food functions after October 15, 2004.
